Submitting a Referral
Are the “Outside Referral Forms” required for referral to Parkland?
Although not required, we highly encourage submission with each referral to reduce and mitigate delays relative to patient identification, financial assistance and desired specialty. View the Patient Access Center Outside Referral Form (OSR).
My organization has a team of referral coordinators. How can they work collectively?
Users from the same organization must request access to view each other’s referrals. Both persons must be on the same domain and must be fully registered. Each person must send requests to refertoparkland@phhs.org authorizing access for the other registrant. Parkland will review and authorize valid requests.
How can I search for patients established at Parkland?
Protection of patient information is paramount. There are two options to align a patient to your referral:
- Click "Select Patient", Input patient’s First Name, Last Name, DOB, Gender, and either the MRN number OR the last four of SSN. If and only if there is an exact match, you can apply the patient information to your record.
- If there is not a match, you can choose "Add Patient" and provide the patient the necessary patient demographics.

How can I see the status of referrals submitted before the portal came online?
Please contact our Patient Access Center at 214-590-5601 for information on any referrals initiated prior to 01/01/2024.
MyChart
My patient would like to access their records electronically. Who can they contact for assistance?

Financial Responsibility
Who can patients contact about financial considerations relative to this referral?
Parkland provides patient estimates for shoppable services. Please contact our Patient Access and Procedure Pricing Inquiry Line at 214-590-8831 for additional questions related to patient estimates for services. Is there a financial office that I can direct patients to?
Parkland’s Patient Financial Services staff is located on the ground level of Parkland's Tower Garage and at Parkland's community clinics to help patients pay for their healthcare. Parkland financial counselors find financial assistance and programs to pay for medical treatment.
Which insurance plans does Parkland accept?
