Parkland reports breach of protected health information

Parkland Health is reporting a breach of protected health information (PHI) involving the Dallas County jail system. Parkland is responsible for providing healthcare to those incarcerated in the county jail.

In September, it was reported that a former Parkland employee sent an email to their personal email address, which contained PHI in the form of a spreadsheet report used for treatment purposes. The PHI included first name, last name, date of birth, and a brief description of the requested exam, treatment, and/or laboratory test. This breach did not impact social security numbers, financial or other account numbers, addresses, electronic medical record systems, or any other personal data.

Following an internal investigation and a law enforcement investigation, Parkland officials determined that the former employee did not intend to remove PHI from Parkland’s environment; and upon discovery, permanently deleted the email and the attachment. It was also determined that the PHI was not retained nor was it further disclosed nor shared with any other party.
